Application Packet

The scholarship application is provided in both Word® document and Adobe® Portable Document Format (PDF).  To read the Adobe PDF document you need Adobe’s PDF Reader (DC), available at no cost.  This Reader will allow you to fill out the application on your computer, tablet, iPad, smartphone, etc.  Similarly, you will need either MS Word® or other document reader such as Google Docs, uTorrent, or other document reader appropriate for your device.

After you have the appropriate tools feel free to download the scholarship application form either as a:

Directions for completing and submitting the scholarship application is in the packet, see above.  Please give yourself ample time to get the application completed, including any personal information, if required, and verification (i.e., DD214 or other valid documentation) of the Vietnam Veteran being interviewed for the essay.  Again, verification can be obtained through a County Veteran Service Officer (CVSO) or Tribal Veteran Service Officer (TVSO) - CVSOs and TVSOs can be located on their website.  Other accredited service officers through Veteran Service Organizations (e.g., American Legion, Veterans of Foreign Wars, Disabled American Veterans, etc.) or a service officer through the Veterans Administration can assist in verifying active military service.
